Grooming Terms and Conditions
Animal Medical West has my permission to do whatever it feels is appropriate for the well-being of my pet. Should Animal Medical West determine that veterinary care is warranted, I understand it is up to my discretion to have my pet seen by a veterinarian, at Animal Medical West or otherwise. *
Animal Medical West will not be held responsible for any conditions or problems discovered during grooming. Should veterinary care be necessary as a result of any services rendered by Animal Medical West, Animal Medical West must be notified within 48 hours of such care. Reimbursement for such care will be at the sole discretion of Animal Medical West. *
Animal Medical West will not be held responsible for clipper burn, minor nicks, or skin irritation resulting from grooming; nor will be we be held responsible for stressful effects that grooming may have upon a pet. Animal Medical West shall not be held liable for any after-grooming effects of de-matting, clipping procedures, or problems uncovered on a badly matted or otherwise neglected coat including but not limited to: itchiness, skin redness, or self-inflicted irritations/abrasions from excessive external rubbing. *
I understand that time and costs associated with de-matting are unpredictable and subject to the condition of my pet; consequently, I agree to pay whatever fees are incurred as a result of de-matting. Whether a pet is "matted" is at the sole determination of Animal Medical West. De-matting is charged a flat fee of $15, an additional $1 per minute will be added after the first 15 minutes. *
I understand that in some instances, de-matting is unfeasible to do and will require shaving. We will attempt to contact you first, but if unsuccessful in our contacting efforts our groomer will determine what is appropriate for the well -being of your pet. If my pet requires any shaving prior to being bathed, I understand there will be a base charge of $15. There may be additional fees depending on time and condition of coat. *
I agree to pay all costs for the grooming of my pet including, but not limited to: special handling requirements, de-matting, and fees due for late drop-off, pick-up, or missed appointments. *
If any fleas or intestinal parasites are found on your pet while here, they will be treated for an additional charge. Grooming prices are dependent on animal size, temperament, and coat condition. All fees are subject to change without notice. *
Animal Medical West reserves the right to refuse to groom any pet for the health and safety of both the groomer and pet. A soft muzzle/basket muzzle may be used, or services discontinued or refused if Animal Medical West determines that a pet presents an unsafe condition. Such conditions include, but are not limited to: behavior issues, health problems, fleas, or parasites. I agree to inform Animal Medical West prior to grooming if the pet has ever bitten any human or another animal, or has any aggressive tendencies whatsoever. I understand and agree that I will be held solely liable for any harm, injury, or property damage caused by my pet. *
If my pet is coughing, vomiting, or having diarrhea, I must notify the staff prior to bringing them in for grooming. Sick pets are not allowed to be groomed and if my pet becomes ill, they will be boarded in the hospital for the remainder of the day which may accrue an additional fee based on the circumstances. *
I agree to be ON TIME or early for my appointment. If I am over 15-20 minutes late, I understand that I may be required to reschedule my appointment and may be charged a missed appointment fee. Any appointment cancelled or rescheduled with less than 24 hours notice may be subject to a $15 late cancellation fee which will be due at the time of cancellation. All No Call/No Shows WILL be assessed a fee equal to half the cost of their appointment, which must be paid in full prior to any further appointments being scheduled. Emergent situations will be considered on a case by case basis. *
If my pet is not picked up by 6:30 pm, I understand I will be charged one night of boarding. The clinic phones go to voicemail after 6:30 pm, therefore if I am not successfully contacted by 6:30 my pet will be required to stay the night. Any fees accrued during this time will be your responsibility and will be payable at time of discharge. *
While we strive to create a safe environment for you pet, there are times when they are not immediately supervised. Please understand that whether or not your pet is being supervised, injuries can and do occur. We do our best to keep these instances to a minimum, but accidents happen. In the event my pet becomes injured during their visit, I hereby authorize any emergency treatment and/or administration of medication deemed necessary by the attending veterinarian. I understand this may include anesthesia. I agree to pay fees for all services rendered at the time my pet is discharged from Animal Medical West. I waive and release Animal Medical West, it's owners, employees, and agents from any and all liabilities of any nature, for injury or damage which I or my pet may suffer including specifically, but not without limitation, any injury or damage resulting from the action of any pet. I expressly assume the risk of any such damage or injury while in a Group Play session or other function of the business, or while on or off of the grounds or surrounding area thereto. *
We require up to date vaccines, a once yearly negative fecal test, and current flea prevention to attend grooming. Your pet must be up to date on these requirements PRIOR to grooming, unless arrangements are made to update on the same day. *
Animal Medical West Grooming hours are by appointment only. Please note that grooming can book out up to 2 weeks in advance. Please schedule ahead or plan accordingly. *
"Express" Service is available at the 7:30 am time slot at no additional cost. These appointments are limited and we suggest scheduling well in advance. "Express" service is available for any appointment time for an additional $25 fee with notice at the time of scheduling. *

Microchip: The microchip is the professional way to identify your pet. A tiny microchip that contains a personal, one-of-a-kind ID number is placed under the skin. This includes a lifetime registration. This is highly recommended for all pets, even if they are indoor only and/or wear a collar. This can be done at any age. There is an additional fee for this service. Would you like to get this done today?

I understand the associated costs of grooming for my pet is a lawful debt. Therefore, I promise to pay the cost of collection, attorney fees, and court costs if such become necessary. I have read and understand the grooming policies of Animal Medical West. By signing this form, I am accepting all risks and responsibility for any costs incurred while my pet is under the care of Animal Medical West.
